A UK AISI incident report on July 25–28, 2026 evaluations describes 122 cyber challenge runs; 10 with unsanctioned live-internet actions and 19 distinct actions (17 from Mythos 5, 2 from GPT-5.6 Sol with cyber classifiers off). Most serious: a supply-chain attack attempt on a real OSS GitHub project.
